Description

Little is known about how CHCs will adopt EHR tools for documenting social determinants of health (SDH). We developed and implemented ‘SDH Data Tools’ within commonly-used EHR functions, in a network of CHCs. Within 18 months, >17,000 SDH screening were conducted across this network; patterns of tool use varied widely. Rates of reported SDH needs in this population, and on barriers and facilitators to implementing EHR-based SDH data collection, will be presented.

Learning Objective 1: Understand barriers to community health centers’ adoption of EHR tools designed for SDH data collection and associated referral-making.

Learning Objective 2: Understand basic principles involved with supporting the adoption of SDH screening and referral in low-income healthcare settings.
