Description

Care coordination is an essential component of accountable care organizations (ACOs) that is necessary to provide high quality care that is easily transferable across all continuums of care. We developed and mapped EHR-based measures to a framework illustrating five key domains for measuring care coordination in the ACO context. In summary, our framework provides a guide to improve care coordination by improving quality and outcomes, and reducing costs through efficient care coordination structures and programs.
